29 CFR 1926.20(b)(2): Such programs had not provided for frequent and regular inspections of the job sites, materials, and equipment to have been made by competent persons designated by the employers. On or about October 3, 2024, Crown Exteriors LLC did not ensure that frequent and regular inspections of the job sites and equipment were made by competent persons designated by the employer, thus exposing employees to the following hazards: B) Nine employees were exposed to fall hazards from installing gutters near unprotected sides or edges and roofing a low-slope roof without adequate fall protection. D) One employee was exposed to struck-by and falling object hazards while working below roofing activities and gutter installation without utilizing head protection.
