29 CFR 1926.760(a)(1):29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: (a) North side of northwest building - On or about October 3, 2024, the employer did not provide any type of fall protection while employees were leaning over the outside edge of the roof attaching the metal roof gutter system exposing employees to fall hazards.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) North side of building - On or about October 3, 2024, the employer had employees use the aluminum extension ladder to gain access to the roof that was not extending past the roofs edge 3 feet exposing employees to fall hazards.
