On or about October 03, 2024, located at 6969 London-Groveport Road, Grove City, Ohio 43123, the employer failed to ensure that the extension ladder was supported on a level surface to prevent accidental displacement when employees used the ladder to access the elevated working surface of the entryway roof of the church building. The condition exposed employees to fall hazards as a result.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. On or about October 03, 2024, located at 6969 London-Groveport Road, Grove City, Ohio 43123, an employee was observed operating a pneumatic nail gun while conducting roofing work without utilizing eye protection. The conditions exposed employees to struck-by hazards as a result. Porfirio Navarrete Alvizar was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1683153, citation number 1, item number 3 and was affirmed as a final order on January 04, 2024, with respect to a workplace located at 7604 Wellshire Drive, Pickerington, Ohio 43147

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about October 03, 2024, located at 6969 London-Groveport Road, Grove City, Ohio 43123, the employer failed to ensure that eight employees were protected from falls while performing roofing work from a two-story church building. The condition exposed employees to fall hazards from approximately 13 feet 11 inches to the ground level. Porfirio Navarrete Alvizar was previously cited for a violation of its equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1683153, citation number 1, item number 4 and was affirmed as a final order on January 04, 2024, with respect to a workplace located at 7604 Wellshire Drive, Pickerington, Ohio 43147.
