Foam Department, Emission Logistics, LLC, 1250 Schweizer Road, Horseheads, NY, 14845, on or about 10/3/2024; Energy control procedures were not utilized by an employee changing the blades on the foam die cutter (Mod. MAV 1ATF, N. M197/3094).  c.) Machine Shop, Emission Logistics, LLC, 1250 Schweizer Road, Horseheads, NY, 14845, on or about 10/3/2024; Energy control procedures were not utilized by an employee changing the dies on the Amada Hydraulic Press Bake (serial number HFBO 125 40 H41122). The employer provided an "Energy Assessment Form" that did not clearly and specifically outline the scope, purpose, authorization, or rules to be utilized for the control of hazardous energy.
