29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about October 3, 2024, employees were exposed to a fall hazard of approximately 7 feet while on standing on a stack of prefabricated walls and rigging them to be lifted by a forklift prefabricated walls because suitable fall protection was not used.   On or about October 3, 2024, employees were exposed to a fall hazard of approximately 22 feet while on the second story of a building unloading prefabricated walls because suitable fall protection was not used.  On or about October 3, 2024, employees were exposed to a fall hazard of approximately 22 feet while throwing pieces of wood from the second story building because suitable fall protection was not used.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person:  On or about October 3, 2024, employees were exposed to fall hazards, electrical hazards and struck by injuries while performing construction activities including using a forklift to lift prefabricated walls because the employer did not ensure a competent person conducted frequent and regular inspections of the worksite and equipment to identify the workplace hazards.  Juan P Villanueva Sagrero was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.20(b)(2) which was contained in OSHA inspection number 1462544, citation number1, item number 1(a) and was affirmed as a final order on July 18, 2024, with respect to a workplace located at 5400 West Landens Way, Peoria, Illinois 61615.
