29 CFR 1926.20(b)(1): The employer did not initiate and maintain a general safety and health program as necessary to comply with this part.  On or about October 3, 2024, the employer did not have a program in place to identify hazards found in the job site, 17415 W State Route 579 in Martin, OH 43445. This program assists employees in maintaining a safe working environment and supports the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls from at least ten feet and the use of personal protective equipment required for job tasks.

29 CFR 1926.503(a)(1): The employer failed to provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about October 3, 2024, the employer, working at the residential address of 17415 W State Route 579 in Martin, OH 43445, did not ensure their employees had been properly trained on how to use appropriate fall protection systems including but not limited to a guardrail system, safety net system, personal fall arrest system, or alternative fall protection measures. Employees were removing parts of an old roof and securing new roofing material to the roof of a two-story residential building without the use of fall protection, exposing employees to falls hazards of  over ten feet.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by wearing protective helmets.  On or about October 3, 2024, the employer of M. Mullet Construction LLC at the worksite located at 17415 W State Route 579 in Martin, OH 43445, did not ensure that employees used protective headgear such as hard hats to protect themselves from overhead hazards from working directly below employees who were using tools and roofing material to attach the roof underlayment to the residential site.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about October 3, 2024, the employer, working at the residential address of 17415 W State Route 579 in Martin, OH 43445, did not ensure their employees were protected from fall hazards using a guardrail system, safety net system, personal fall arrest system, or an alternative fall protection measure.
