29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. Location - 53 Hildreth Street, Lowell, MA 01850. On or about October 4, 2024, two employees were using two sets of pump jack scaffold systems that did not have bottom support brackets.

Location - 53 Hildreth Street, Lowell, MA 01850. On or about October 4, 2024, the employer was using a waist belt to tie into a lifeline that did not have a D-Ring at center of wearers back.
