At a temporary worksite located at 4570 Palm Valley Road, Ponte Vedra Beach, Florida 32082: On or about October 4, 2024, the employer exposed employees to fall hazards, in that, the side rails of a portable ladder used to gain access to the roof did not extend three feet above the roof when employees were accessing or egressing the roof.
