29 CFR 1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. Location: Warehouse West Side Instance 1: On or about October 4, 2024, employees were exposed to trip and fall hazards due to metal plates on the floor in the aisles and in front of Overhead Door 1, Overhead Door 2, and Overhead Door 4. Instance 2: On or about October 4, 2024, employees were exposed to trip and fall hazards due to debris and wood on the floor in front of two double exit doors located on west wall. Location: Warehouse East Side Instance #3: On or about October 4, 2024, employees were exposed to trip and fall hazards due to metal, and debris on the floor in front of two double exit doors located on the east wall. Instance 4: On or about October 4, 2024, employees were exposed to trip and fall hazards due rolls of band saw blades, five gallon metal buckets containing Metal Adhesive, and debris on the floors of the East Side Storage Room.

29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: Location: Warehouse West Side Instance 1: On or about October 4, 2024, employees were exposed to smoke inhalation and burns due to a portable fire extinguisher mounted on a wall west of an exit door located by Overhead Door #4 was blocked by a 2" X 4" four foot section of lumber. Instance 2: On or about October 4, 2024, employees were exposed to smoke inhalation and burns due to a portable fire extinguisher mounted on a west side wall, behind a Hyd-Mech V18 Series II Band Saw #3, being blocked by wood on the floor, a wooden pallet containing a large cardboard box, and a wooden pallet containing cardboard. Location: Warehouse East Side Instance 3: On or about October 4, 2024, employees were exposed to smoke inhalation and burns due to a portable fire extinguisher mounted on an east side wall being blocked by metal, a broken white plastic five gallon bucket, and debris on the floor.
