(a) On or about 9/26/2024, and at times prior, the employer, Ridgeline Roofing Services, LLC., did not ensure frequent and regular inspections of the jobsite located at 5583 South Jebel Way, Centennial, CO 80015 were conducted. Sub-contracted employees were observed conducting reroofing construction without the use of fall protection and eye protection.
