29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): The employer did not ensure that employees were protected from elevated fall hazards. The condition was observed on or about 04 October 2024, when employees were observed roofing a residential building at 1046 Powell Street in Saint Charles, Missouri. The employer did not utilize guardrail systems, a safety net system, or a personal fall arrest system. Two employees were observed working on the roof of the building, at heights approximately 15-20 feet and greater above the ground, without fall protection and thus were exposed to fall hazards.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: The employer did not ensure that employees were protected from elevated fall hazards. The condition was observed on or about 04 October 2024, when employees were observed roofing a residential building at 1046 Powell Street in Saint Charles, Missouri. The employer did not secure the aluminum extension ladder on the roof which was not level and being used to gain access to the second level of the structure. Two employees were observed using the unsecured ladder.
