29 CFR  1910.29(e)(1): 29 CFR 1910.29(e)(1): The employer did not ensure that each cover for a hole in a walking-working surface is capable of supporting without failure, at least twice the maximum intended load that may be imposed on the cover at any one time:  (a)  Well Pad located at 57 Field & Stream Road, Marianna, PA:  On or about October 1, 2024, there was no substantial cover over the 8 foot in diameter, 8 foot deep 7H well cellar.  The 7H well head was installed in the 7H well cellar, and it was several inches below grade.  The 8-foot diameter 7H well cellar hole was covered with one layer of (600x geotextile 16 ounce felt paper) felt paper.    The employer had 4 orange cones placed around the 8-foot diameter opening to the 7H well cellar as a warning to employees.
