29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  (a) Workroom floor: On or about 10/7/2024 and at times prior to, the employer exposed employees to struck by hazards, in that forklifts training was not provided to employees operating forklifts while moving and other PIT related hazards, in that forklift operators were observed driving PITs in the same area where employees were walking and working, and had not been evaluated as required.  U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.178(l)(4)(iii), which was contained in OSHA inspection number 1698831 citation number 1, item number 1 and was affirmed as a final order on 1/31/2024, with respect to a workplace located at 1150 N. McDowell Blvd, Petaluma, CA 94999.  U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.178(l)(4)(iii), which was contained in OSHA inspection number 1676941 citation number 1, item number 1 and was affirmed as a final order on 11/08/2023, with respect to a workplace located at 615 6th Ave Se Cedar Rapids, IA 52401.
