29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about October 07, 2024, during a commercial construction roofing project located at 12782 ROCKSIDE RD, GARFIELD HEIGHTS, OH 44125, the employer did not ensure employees working from a low-sloped roof were protected from fall hazards by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.

29 CFR 1926.502(h)(1)(i):The safety monitor shall be competent to recognize fall hazards: On or about October 07, 2024, during a commercial construction roofing project located at 12782 ROCKSIDE RD, GARFIELD HEIGHTS, OH 44125, the employer did not ensure that the safety monitor was competent to recognize the need to utilize a combination warning line system and a safety monitoring system on a roof that was greater than 50-feet in width.
