29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)    a. On or about October 7, 2024, at the above addressed jobsite, an employee performing residential carpentry activities, twelve (12') feet four (4") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about October 7, 2024, at the above addressed jobsite, an employee performing residential carpentry activities, twenty-one (21') feet seven (7") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  Serrano Carpentry Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1643684, citation number 1, item number 2 and was affirmed as a final order on December 6, 2023, with respect to a workplace located at 145 S. Lawndale Ave., Elmhurst, IL 60126.

29 CFR 1910.178(l)(6):Certification. The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a. On or about October 7, 2024, at the above addressed jobsite, the employer failed to keep a written certification record of training regarding the operation of a Genie GTH-844 8042 Telehandler.
