Employees were using a closed 6-foot A-frame ladder, propped against the loading dock.  b)  Jobsite located at 500 Christopher Columbus Blvd. Philadelphia PA 19123. On or about October 7, 2024, the employer did not ensure an employee was using a ladder in the manner in which it was designed. One employee was using a closed 10-foot A-frame ladder, propped against the side of the loading dock to access the scaffold.
