29 CFR  1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy:  On or about October 8, 2024, at a facility located at 4315 Transport Drive in Weston, WI; an outlet receptable box and an outlet light switch box did not have a cover, faceplate, or fixture canopy to protect employees from shock hazards presented by the live electrical components in the outlet boxes.

29 CFR  1910.1200(f)(6)(ii):  The employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  On or about October 8, 2024, at a facility located at 4315 Transport Drive in Weston, WI; the employer did not ensure a portable pressure sprayer containing Blue Degreaser was labeled with the product identifier or information regarding the hazards of the chemical.  The employer did not ensure a containers of diluted cleaning chemicals located in the center wash bay were labeled with the product identifier or complete information regarding the hazards of the chemical due to the labels on two of the containers had been damaged or washed-out to the point of being illegible.  The Nova Wash LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(f)(6)(ii), which was contained in OSHA inspection number 1682469, citation number 1, item number 8 and was affirmed as a final order on October 18, 2023, with respect to a workplace located at 4315 Transport Drive, Weston, WI 54476.

29 CFR  1910.146(d)(14):  Review the permit space program, using the canceled permits retained under paragraph (e)(6) of this section within 1 year after each entry and revise the program as necessary, to ensure that employees participating in entry operations are protected from permit space hazards:  On or about October 8, 2024, at a facility located at 4315 Transport Drive in Weston, WI; the employer had not reviewed canceled permits from the prior year to ensure that employees participating in entry operations were protected from permit space hazards.
