Location: Spin forge machine a) On or about 10/4/2024, a work-specific lockout/tagout procedure for the installation of the spin forge machine, to include positioning, jogging, and troubleshooting of the spin forge trolley was not developed or utilized.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof. Location Spin forge machine On or about 10/4/2024, an employee engaged in testing and troubleshooting the machine trolley was not removed from the machine danger zone, which comprised the space in and around the spin forge trolley back plate and the spin forge trolley. Employee suffered multiple finger amputations when the trolley jogged unexpectedly backwards crushing the employee's hand/fingers.
