Location Spin forge machine   On or about 10/4/2024, an employee engaged in testing and troubleshooting the machine trolley was not removed from the machine danger zone, which comprised the space in and around the spin forge trolley back plate and the spin forge trolley. Employee suffered multiple finger amputations when the trolley jogged unexpectedly backwards crushing the employee's hand/fingers.
