29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about October 4, 2024, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.  On or about October 4, 2024, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet four (4) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about October 4, 2024, at the above addressed jobsite, an employee performing residential roofing operations, twelve (12) feet above ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about October 4, 2024, at the above addressed jobsite, employees performing residential roofing operations, ten (10) feet two (2) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a.  On or about October 4, 2024, at the above addressed jobsite, the portable ladder used to gain access to the roof did not extend at least three (3) feet above the roof's surface, thereby exposing the employees to fall hazards.
