29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: (a)Indian Health Services dba Pine Ridge Service Unit at 607 Indian Health Road, Pine Ridge, SD 57770: On or about and at times prior to October 3, 2024, the employer did not ensure that the Electrical Breaker, Number 1 contained within NHBI Electric Panel, Asset Number: NHBI from MDB-4, Serial Number: 10605-A1 was deenergized prior to maintenance personnel removing and installing facility lights. This condition exposed employees to shock and electrocution hazards.
