29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: On or about September 20, 2024, and at times prior thereto, in the north section of the shingles building, an adequate protective system was not provided while employees cleaned buildup off star wheels and backstops, in the following instances (a-d): a) Employees were not provided an adequate protective system while cleaning auto catcher #1. b) Employees were not provided an adequate protective system while cleaning auto catcher #2. c) Employees were not provided an adequate protective system while cleaning auto catcher #3. d) Employees were not provided an adequate protective system while cleaning auto catcher #4.
