29 CFR  1910.134(a)(2):A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee:  Employees working in the Evergreen Room with the following overexposures were not provided a respirator.   a) Evergreen Room: an employee working in the Evergreen Room, maintaining the production line are exposed to an airborne concentration of Hydrogen Peroxide at an 8 hour time-weighted average of  2.7525 mg/m3, which was 1.97 times the permissible exposure limit (PEL) of 1.4 mg/m3. The sampling was performed for 367 minutes during one shift on 12/3/24. Zero exposure was assumed for the un-sampled period of time.   b) Evergreen Room.  An employee working in the Evergreen Room maintaining the production line was exposed to an airborne concentration of hydrogen peroxide at an 8 hour time-weighted average of 2.4287 mg/m3, which was 1.74 times the permissible exposure limit (PEL) of 1.4 mg/m3.  The sampling was performed for 402 minutes during one shift on 12/3/24. Zero exposure was assumed for the un-sampled period of time.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
