29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about October 8, 2024, a portable ladder with a physical defect was not taken out of service immediately and was still in use by employees.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 8, 2024, employees did not utilize a fall protection system when at heights six (6) feet or greater above a lower level while performing roofing work.   Renee Sifuentes Luis was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1728847, citation number one, item number one and was affirmed as a final order on June 18, 2024, with respect to a workplace located at 1241 Greenfield Road in Cedar Hill, TX.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 8, 2024, employees engaged in roofing work on a residential home used extension ladders that did not have the side rails extended three (3) feet above the upper landing surface in the following instances (a-c):  a) Portable ladder on the West Side of the home. b) Portable ladder on the South Side of the home. c) Portable ladder on the East Side of the home.  Renee Sifuentes Luis was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1728847, citation number one (1), item number two (2) and was affirmed as a final order on June 18, 2024, with respect to a workplace located at 1241 Greenfield Road in Cedar Hill, TX.
