Eagle Construction, LLC at 147 Chisolm Drive, Box Elder, SD 57719: On or about and at times prior to October 8, 2024, the employer did not ensure employees were protected from falls by a personal fall arrest system while conducting roofing activities and working from a JLG Aerial lift Platform, Model Number 660SJ US, Serial Number 0300268093. This condition exposed employees to fall and ejection hazards while working from the aerial lift.

Eagle Construction, LLC at 147 Chisolm Drive, Box Elder, SD 57719: On or about and at times prior to October 8, 2024, the employer did not ensure that employees were protected from falls while conducting roofing operations on a low slope commercial property. This condition exposed the employees to fall hazards greater than six feet.
