29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:    a) Job site, 3717 California Avenue, Pittsburgh, PA: On or about October 8, 2024, during siding work, the employer did not ensure that an employee on the main house roof used appropriate fall protection systems/devices where exposed to falls of up to approximately 20 feet.   b) Job site, 3717 California Avenue, Pittsburgh, PA: On or about October 8, 2024, during roofing work, the employer did not ensure that an employee on the rear porch roof used appropriate fall protection systems/devices where exposed to falls of up to approximately 12 feet.   c) Job site, 3717 California Avenue, Pittsburgh, PA: On or about October 8, 2024, during roofing work, the employer did not ensure that an employee on the garage used appropriate fall protection systems/devices where exposed to falls of up to approximately 12 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:     a) Job site, 3717 California Avenue, Pittsburgh, PA: On or about October 8, 2024, during roofing and siding work, the employer did not ensure that ladders used at the job site extended at least 3 feet above the landing in that ladders used at the site ranged from about 18 to 30 inches above their respective landings.

29 CFR 1926.20(b)(2):  The employer did not have a program that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons:    a) Job site, 3717 California Avenue, Pittsburgh, PA: On or about October 8, 2024, during siding and roofing work, the employer did not ensure that a system was in place to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, ladders not being extended properly above their landings and failing to use eye/face protection when using nail guns.
