Front Entrance Way, 1620 N Tuckahoe Street, Bellwood, PA 16617: on or about October 4, 2024, An employee was working from a work platform that was not firmly secured to the lifting carriage and/or forks of a Hyster 90 - 1049 Fortis Forklift brand powered industrial truck (model number: H90FT, serial number U005V04747R) through the use of fall protection per the requirements of the standard, guardrails 48 inches in height.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a.) Front Entrance Way, 1620 N Tuckahoe Street, Bellwood, PA 16617: Observed on or about October 4, 2024, a work platform used with a hyster 90-1049 Fortis forklift to lift personnel was not approved by the manufacturer. Employees using the Sidasu Safety Cage work platform to perform building maintenance or to secure signs from the upper level of the front entrance way of the building were exposed to a fall hazard of approximately 12 feet to the concrete ground below.
