29 CFR 1910.268(g)(1):The employer did not ensure the proper use of a positioning system or a personal fall arrest system while work is performed at positions more than 4 feet (1.2 m) above the ground on poles and on towers: Employee was exposed to fall hazards. This was documented on or about October 8, 2024, at North of Hwy 91 & 280th Ave., Humphrey, Nebraska. An employee climbed a a telecommunications tower approximately 17 feet from the ground without the use of fall protection.
