TD IS, s/n: 1031, in that guarding had been removed, exposing the intake chain and conveyor system.  This condition exposed employees to pinched-by, caught-in, and laceration hazards.

(b)Dakota Tom's, Inc. at 270 East Main Street, Corsica, SD 57328: On or about and at times prior to October 8, 2024, the employer did not ensure that damaged electrical equipment was repaired/replaced in that an outlet cover plate on the south wall of the "Cheese Side" was missing.
