Machinery  includes, but is not limited to, PTI Packline, GIRO Girbagger smart system Model UBS65, Static Weighter, Daumar Type CEA-55, occurred on or about 10/08/2024.  a) S&L Cold Storage Inc. -  Sanitation department: The employer did not establish a Lock Out/Tag Out Program consisting of energy control procedures, employee training, and periodic inspections to protect employees against the unexpected energization of equipment when performing cleaning and sanitation in machinery including but is not limited to, PTI Packline, GIRO Girbagger smart system Model UBS65, Static Weighter, Daumar Type CEA-55, occurred on or about 10/08/2024.

29 CFR  1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:   a) S&L Cold Storage Inc. -  Packing area: The employer failed to ensure that employees were properly securing, interlocking, and limiting the height of loads on forklifts to prevent sliding, tilting, or collapsing. On 9/11/24 an 8 foot high stack of product fell onto an employee as the forklift operator was turning the lift, the employee was taken to the emergency room.

The charging cables on the ECOTEC Model# ACCESS 13/36, S/N 23C7457 battery had damaged cords with damaged outer casing, observed on or about 10/08/2024.

Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.   a) S&L Cold Storage Inc. -  Throughout the manufacturing area:  Pallets of finished product were stacked around the work areas blocking the view of the forklift operators to employees working between the stacks of product and the packing lines.  Occurred on or about 10/08/2024.  b) S&L Cold Storage Inc.-Throughout the manufacturing area: Employees are exposed to struck by hazards due to empty boxes and trash stacked around the works areas blocking the view of the forklift operators to employees working between the stacks of boxes and trash and the packing lines.   Occurred on or about 10/08/2024.
