OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck-by hazards from high pressure equipment.  On or about October 4, 2024, the employer failed to protect employees from struck-by hazards associate with high pressure pump use during well servicing operations.  a)	Employees were exposed to struck-by hazards when the pressurized hoses and lines attached to the Gardner Denver mud pump were not secured to prevent unsafe movement and whipping. b)	Employees were exposed to struck-by hazards when the Gardner Denver pump became pressurized resulting in the failure of the Balon valve connection.  This failure caused the valve to separate from its connection. The valve and connected components became airborne striking an employee.
