1910.178(l)(1)(i) The employer did not ensure that powered industrial truck operator training adequately addressed pedestrian awareness hazards and the safe interaction between pedestrians and powered industrial truck operations, including operations in dock areas where visibility may be restricted. On or about October 4, 2024, at Dock 95, employees working as pedestrians in areas where powered industrial trucks were operating were exposed to struck-by hazards during forklift loading operations involving oversized freight.
