The employer failed to establish and implement a written respiratory protection program with worksite specific procedures for required respirator use. (b) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to provide procedures for medical evaluations to employees required to use the respirators as required. (c) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to provide fit testing procedures for tight-fitting respirators to employees required to use the respirators.
