29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   (a) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to establish and implement a written respiratory protection program with worksite specific procedures for required respirator use.  (b) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to provide procedures for medical evaluations to employees required to use the respirators as required.  (c) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to provide fit testing procedures for tight-fitting respirators to employees required to use the respirators.  (d) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators.  (e) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer failed to provide procedure for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators.  (f) 707 Plane Street, Fabrication Shop (Unit A5) GYPSUM, CO 81637: On or around October 9, 2024, and at times prior, employees were exposed to respiratory hazards while operating the CNC machine and using hand cutting tools. The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program.
