29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Job site, 110 through 112 Andrews Drive, Robinson Township, PA: On or about October 9, 2024, during roofing activities, the employer did not ensure that an employee was protected while working outside the flag line in that he was not tied off and was not protected by guardrails, a safety monitor or safety nets, where exposed to a fall hazard of up to approximately 19 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems were not rigged such that an employee could neither free fall more than 6 feet (1.8 m), nor contact any lower level:    a) Job site, 110 through 112 Andrews Drive, Robinson Township, PA: On or about October 9, 2024, during roofing activities, the employer did not ensure that fall arrest systems were rigged such that an employee could not fall more than 6 feet and could not contact a lower level in that an employee was tied off outside the flag lines but the system was utilized in such a manner that he could have fallen and struck the ground below where he worked, exposing him to a fall hazard of approximately 19 feet.

29 CFR1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected not less than 6 feet (1.8 m) from the roof edge which was parallel to the direction of mechanical equipment operation, and not less than 10 feet (3.1 m) from the roof edge which was perpendicular to the direction of mechanical equipment operation:    a) Job site, 110 through 112 Andrews Drive, Robinson Township, PA: On or about October 9, 2024, during roofing activities, the employer did not ensure that flag lines were kept at least 10 feet (3.1 m) from the roof edge that was perpendicular to the direction of mechanical equipment operations, exposing employees to fall hazards of approximately 19 feet.
