29 CFR 1903.2(a)(1): The employer did not post and keep posted OSHA notices informing employees of the protections and obligations provided for in the Act, and that for assistance and information, including copies of the Act and of specific safety and health standards, employees should contact the employer or the nearest office of the Department of Labor. Such notice or notices shall be posted by the employer in each establishment in a conspicuous place or places where notices to employees are customarily posted. Each employer shall take steps to ensure that such notices are not altered, defaced, or covered by other material: On or about October 10, 2024 and at times before, the employer did not post and keep posted OSHA notices informing employees of the protections and obligations provided for in the Act. The employer failed to have any OSHA poster available for employees.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about October 10, 2024 and at times prior, the employer did not establish an energy control program to include energy control procedures and affected employee training prior to service and maintenance activities being conducted at the facility.
