29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): At 615 Old St Mary's Rd, Perryville, MO, 63775, On September 7, 2024 and at times prior thereto, the employers energy control procedures, such as but not limited to, Manufacturing line 2, Cooker 2 did not contain the required information or it was contradictory to other information available and did not contain the necessary information for verification. A. 29 CFR 1910.147(c)(4)(ii)(B), Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The procedures did not provide steps for shutting down the machines nor did they identify all sources of hazardous energy, such as air, steam, and hydraulic energy sources. B. 1910.147(c)(4)(ii)(D), Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: The procedures did not address specific actions to perform for verification of the effectiveness of the lockout implementation and other energy control measures. The Gilster-Mary Lee Packaging Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1650556, citation number 1, item number 1 and was affirmed as a final order on August 25, 2023, with respect to a workplace located at 2301 S Wabash, CENTRALIA, IL 62801 .
