(a) Montgomery Bridge Project: On October 8, 2024, an employee was operating a Genie S-65 XC telescopic boom lift and was not wearing fall protection, exposing the employee to a fall of up to approximately 20ft. to the ground below.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service. (a) Montgomery Bridge Project: On October 8, 2024, the personal fall protection body harness and 6-ft. lanyard provided by the employer for use when operating and/or working in the Genie S-65 XC telescopic boom lift was identified as being defective and was not removed from service prior to use. The harness's dorsal D-ring and the lanyard's snaphook had rusted/corroded metal parts, causing the gate of the lanyard snaphook to get stuck and not fully close, thereby exposing employees to falls of up to 20ft. to the ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Montgomery Bridge Project: On October 8, 2024, employees were working at heights of up to 23 feet 5 inches above grade while installing platform fencing on the underside of a bridge. The employees were not using fall protection, leaving them unprotected from potential falls to the ground.
