29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: Hallway Infront of Room 4226 next to staircase: On or about October 10, 2024, and at times prior thereto, a ceiling tile had a 2-3 inch hole. This condition would delay the activation of the sprinkler system(s) preventing a proper barrier and restricting the movement of smoke in accordance with NFPA 101 8.5.1. This condition potentially exposed employee(s) to fire and smoke hazard(s).
