29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) On or about October 10, 2024, at the job site located at 150 Lake Street, Pewaukee WI 53072, the employer had not ensured that employees tasked with a water main tie-in had adequate protective system(s) in place prior to entering a 6.5-foot trench, exposing the employees to engulfment hazards.
