29 CFR 1910.179(j)(2)(i): All functional operating mechanisms were not inspected daily for maladjustment interfering with proper operation. a. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about September 30, 2024, the employer did not ensure daily functional tests of Crane #2, an Engineered Material Handling overhead gantry crane (model ZLK; s/n 975382), were conducted. Employees were exposed to struck-by and crushing injuries in that daily checks of functions such as, but not limited to, limit switches, wire rope damage, pendant controls, brakes, and hooks, were not performed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed. a. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about September 30, 2024, the employer did not ensure that unsafe conditions identified during the annual inspections of Crane #1, a floor-operated Shepard Niles overhead gantry crane (s/n 70260B), were corrected prior to continued use. Inspections disclosed that the bridge and trolley lacked rail sweeps to prevent damage to wheels and derailment. Employees who operated the crane to move machine molds were exposed to struck-by and crushing injuries. b. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about September 30, 2024, the employer did not ensure that unsafe conditions identified during the annual inspection of Crane #2, a floor-operated Engineered Material Handling overhead gantry crane (model ZLK; s/n 975382) were corrected prior to continued use. Inspections disclosed that the radio-controlled pendant was missing function labels, and the wire rope dead end required redress to prevent slippage. Employees who operated the crane to move machine molds were exposed to struck-by and crushing injuries. c. ABC Technologies WMG USA, Inc. - Bellevue, Ohio: On or about September 30, 2024, the employer did not ensure that unsafe conditions identified during the annual inspections of Crane #4, a floor-operated Engineered Material Handling overhead gantry crane (s/n 102016) were corrected prior to continued use. Inspections disclosed that the wire rope was incorrectly installed in the wedge socket. Employees who operated the crane to move machine molds were exposed to struck-by and crushing injuries. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The control buttons for hoisting and lowering were not labeled to indicate the direction of movement. No additional abatement information is needed for this item.
