29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at 16120 Ryan Cr, Parkville MO -- employees engaged in roofing duties using pneumatic nail guns, hammers and various small hand tools were not using any eye protection. This exposed the employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 10/7/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a new residential worksite located at 16120 Ryan Cr, Parkville MO -- employees engaged in roofing duties were working from heights greater than 25' above ground level without fall protection. This exposed the employees to fall hazards causing serious injury and/or death. This was documented on or about 10/7/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a new residential worksite located at 16120 Ryan Cr, Parkville MO -- employees engaged in roofing duties were using 2 extension ladders to gain access to the roof. The 1st extension ladder from the ground to the lower roof was tipped over and not upright against the building. The 2nd ladder going from the lower level to upper level of building did not extend 3 feet above the landing surface. This exposed the employees to fall hazards. This was documented on or about 10/7/2024.
