A) 34 Alida Street, Yonkers - NY:  Employees were exposed to fall hazards of approximately 13-feet while working on a pump jack scaffold with no fall protective devices, on or about, October 6th, 2024.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   A) 34 Alida Street, Yonkers - NY: Employees were exposed to fall hazards of approximately 12 1/2- feet while using an extension ladder to access a scaffold. The extension ladder that was used was not extended at least 3 feet above the surface in which they were accessing, on or about, October 6th, 2024.
