(b) Throughout the site, located at 1855 Tileston Road, Saint Cloud, Florida, 34771: On or about 10/15/2024, employees operating the Nissan APJ02A25V serial # APJ02-9U1518, powered industrial truck were exposed to crush-by hazards as the forklifts lacked operator restraint systems, such as seatbelts.
