29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) First Floor Roof: Hotel Indigo, 39 East Gay St, West Chester, PA 19380 - On or about October 11, 2024, the employer failed to ensure that employees who were working on the roof of the building were protected from an uncovered 50 inch x 50 inch roof hole, thereby exposing employees to falls of approximately 17ft.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) First Floor Roof: Hotel Indigo, 39 East Gay St, West Chester, PA 19380 - On or about October 11, 2024, the employer failed to ensure that employees installing insulation on the roof were provided with a training program on fall protection before working near an uncovered hole, thereby exposing employees to falls of approximately 17ft.
