OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught-between hazards while moving a large, palletized crate of glass panels. LOCATION: The storage yard at 201 Commerce Dr., Braintree, MA On or about October 11, 2024, employees were exposed to struck by and crush by hazards when off-loading crates of glass panels from a tractor trailer shipping container with a forklift. The employer did not establish a work zone/boundary zone to prevent employees performing spotter operations from struck-by and crush-by hazards.
