29 CFR 1926.300(b)(3): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Jobsite: On or about October 15, 2024, an employee was not protected from inadvertently reaching through the guard (approximately 4-inch square) openings to the gyp create line pump mixer (Strong Manufacturer) and coming in contact with the rotating metal mixing paddles of the machine.

29 CFR 1926.200(c)(1): Caution signs were not used only to warn against potential hazards or to caution against unsafe practices: (a) Jobsite: There were no caution sign(s) on the gyp create mixing machine cautioning employees to keep their hands out of the area of the rotating metal paddles that mixed the gyp create. In addition, many of the decals (indicating operations or warnings or operations of the machine) were either partially worn off or completely illegible.
