29 CFR1910.178(a)(4):  Modifications or additions which affect capacity and safe operation of a powered industrial truck were performed by the employer without the manufacturer's prior written approval.  Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.  Location: 50 North Plains Industrial Road Wallingford, CT 06492  On or about October 16, 2024 and times prior to,  the employer modified a CAT powered industrial truck's capacity and safe operation by adding a CONTRX telescoping jib for material handling without the manufacturer's prior written approval exposing employees to rollover and struck-by hazards.

Location: 50 North Plains Industrial Road Wallingford, CT 06492  On or about October 16, 2024, and times prior to, employees were exposed to caught-in and amputation hazards operating the pinning machine with an unguarded horizontal driveshaft.
