29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a. On or about October 16, 2024, at the above addressed jobsite, an employee working on the roof, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)   a. On or about October 16, 2024, at the above addressed jobsite, employees performing residential roofing operations, seven (7) feet nine (9) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Falcon Construction & Builders Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1701692, citation number 1, item number 2 and was affirmed as a final order on November 21, 2023, with respect to a workplace located at 2778 Cranston Cr. Yorkville., Illinois 60560.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will, not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a. On or about October 16, 2024, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.  Falcon Construction & Builders Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1701692, citation number 1, item number 4 and was affirmed as a final order on November 21, 2023, with respect to a workplace located at 2778 Cranston Cr.
