29 CFR 1910.303(b)(7)(iv): There existed damaged parts that could have adversely affected the safe operation or mechanical strength of electrical equipment, such as parts that were broken, bent, cut: On or about October 16, 2024, on the side of the Company Man's trailer, employees utilized the trailer which had visible damage (split open) on the PVC LB type Conduit Body casing high voltage electrical conductors used to supply power to the trailer during oil and gas operations.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about October 16, 2024, employees utilized trailers for working and sleeping quarters with deficient 2-outlet receptacles in the following instances. a) On the side of the Company Man's trailer (#TA1002) the 2-outlet receptacle was missing a face plate. b) On the side and backside of the Company Man's sleeping quarters trailer (#TA1017) four 2-outlet receptacles were missing face plates. c) On the side of the Directional Driller's trailer (#TA1007) two 2-outlet receptacles were missing face plates. d) On the side of the Night Company Man's trailer (#TA1060) the 2-outlet receptacle was missing a face plate. e) On the side of the Solid Control trailer (#TA1008) the 2-outlet receptacle was missing face plate.
