On or about October 16, 2024, at the Mud Logger's work trailer, employees utilized an electrical extension cord which was run through the side window to operate the Mud Logger's test pump(s).

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location: On or about October 16, 2024, on the inside of the Mud Logger's work trailer, employees utilized a sink (water source) in close proximity to an electrical outlet receptable which was not protected by GFCI.
