:29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface: a) Jobsite located at or near 19 Lukens Mill Drive, Coatesville, PA - On or about October 16, 2024, the employer did not ensure that portable ladders used to gain access to the roof were extended at least 3 feet above the upper landing surface. The employee used an extension ladder to gain access to the roof to perform gutter cleaning without the ladder extended at least 3 feet above the upper landing surface, thereby exposing employees to fall hazards of approximately 20 feet.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Jobsite located at or near 19 Lukens Mill Drive, Coatesville, PA - On or about October 16, 2024, the employer did not ensure that an employee working adjacent to unprotected edges was protected from falls by the use of guardrail system, safety net system, or personal fall protection system. An employee performed gutter cleaning on the sloped roofs of town homes without the use of fall protection, thereby exposing the employee to falls approximately 20 feet from the ground level.
